Option 2 Plan

For a one time fee, the Option 2 Plan provides a complete in-vitro fertilization cycle including embryo transfer with the option of a second complete IVF cycle if the first cycle is not successful. This plan also covers the transfer of cryopreserved (frozen) embryos within the twelve month period of the plan. This plan is not applicable to the use of an ovum donor or a gestational carrier.

In-cycle services provided under plan:

  • medical services provided by a PFC physician in our facilities, including clinical monitoring, egg retrieval, embryo transfer and in-cycle medical consultations related to IVF
  • basic services of the IVF laboratory, including the in-vitro fertilization procedure, and cryopreservation and storage of embryos for one year
  • subsequent transfer of any frozen / thawed embryos within the one-year period

Costs not covered under plan:

  • pre-cycle evaluation and lab testing
  • medications for ovarian stimulation and preparation of the uterus for optimal implantation
  • anesthesia during the egg retrieval procedure
  • facilities fee
  • pregnancy tests; obstetrical ultrasound

Special services not covered (not required by all patients):

  • services of a non-PFC physician, such as an urologist performing a Testicular Sperm Extraction (TESE) procedure or an obstetrician monitoring a patient outside of a PFC facility
  • any other medical treatment or monitoring provided by a non-PFC physician and / or facility
  • Intracytoplasmic Sperm Injection (ICSI) or other specialized laboratory services
  • donor sperm

Medical Criteria:

  • IVF/ET must be medically indicated.
  • The female partner must be under 39 years of age at the commencement of the first fresh cycle of treatment. The date of commencement of treatment is determined by the start of the fertility medications used to stimulate the production of multiple follicles.
  • The female partner must meet medical criteria based upon review of past medical history, physical examination, precycle lab work including measurement of FSH and estradiol, and documentation of a normal uterine cavity, free of pathology.
  • An appropriate sperm source must be provided by the individual or couple.
  • The couple or individual must comply with all medical protocols and directives related to treatment prescribed by the PFC physician.
  • The couple or individual must sign all informed consent and financial agreement documents.

Option 2 Plan Fees

$12,150 (own eggs)

Included in services:

  • Up to two (2) fresh cycles of IVF
  • Thawed embryo transfer cycles to use all cryopreserved embryos
  • Cryopreservation for one (1) year

Not included: precycle screening, medications, anesthesia, facilities charges, and when applicable, ICSI fees or other specialized laboratory services.

NOTE: Must meet inclusion criteria to participate in this plan.

All fees are subject to change without advance notice. If the couple or individual and PFC have executed a financial agreement, the price stated in the agreement will be honored for the cycle, regardless of any change in fees occurring after the document was signed. Prices effective 1/25/00
